Alert 03 – 13 Improper Position of Helper Results in Lost Time Incident
WHAT HAPPENED:
Forklift operator and helper are trying to get this kelly (pipe) into the casing. Helper is giving hand signals to driver. The pipe rolled off the forks and hit the helper’s foot. The helper’s foot and ankle were broken. His colleagues had to lift the pipe off his foot. He will be off work for at least 4 weeks.
